Almost everyone who says “it never comes out the same at home” is making two or three of the mistakes on this list, and none of them are about the machine. The quality of a cup depends on four variables —grind, dose, temperature and time— and all four are free to fix.
Here are the twelve most common mistakes ordered by impact, with the exact correction and the concrete figures for each method. Apply the first five and you will notice the difference tomorrow morning.

If your coffee comes out bitter, you have over-extracted: grind coarser, lower the temperature or shorten the time. If it comes out sour and watery, you have under-extracted: grind finer, raise the temperature or extend the time. Change one variable at a time.
Coffee and grind mistakes
1. Using pre-ground coffee from weeks ago
Ground coffee loses most of its aromatic compounds in the first 15-20 minutes. A bag opened a month ago is, aromatically speaking, brown water. The fix: buy whole beans and grind just before brewing. It is the highest-impact change on the whole list.

2. Blade grinder
A blade grinder does not grind: it chops at random. It produces dust and coarse chunks at once, and the dust over-extracts (bitter) while the chunks under-extract (sour). Both defects in the same cup. The fix: conical or flat burrs, even if hand-cranked.
3. The wrong grind for the method
Each method needs its own size because the water is in contact with the coffee for a different length of time:

4. Buying torrefacto coffee
If sugar appears in the ingredients, the bean was roasted with added sugar. It turns bitter, leaves burnt residue and fouls the grinder. The fix: look for "natural roast" and, better still, a roast date on the bag.
Water and temperature mistakes
5. Boiling water
At 100 °C bitter compounds are extracted that at 93 °C stay where they are. The fix: 90-96 °C. Without a thermometer: boil and wait 30-45 seconds with the lid open.

6. Hard tap water
Coffee is 98% water. Water with too much limescale mutes the acidity and gives a flat cup, as well as wrecking the machine. The fix: filtered or lightly mineralised water (dry residue below 150 mg/l). Distilled water is no good either: with no minerals there is no extraction.
7. Preheating nothing
Pouring coffee at 92 °C into a cold cup takes 8-10 °C off it in two seconds. The fix: rinse cup, jug and portafilter with hot water before you start. It takes ten seconds.
Dose and ratio mistakes
8. Measuring by spoonfuls
A scoop of light-roast coffee weighs up to 3 g more than a dark-roast one because of bean density. So the same gesture gives you very different coffees. The fix: scales.

9. Ignoring the ratio
The coffee-to-water ratio is the main lever on the result:

10. Tamping the coffee in the moka pot
The moka pot works at 1,5-2 bars, not 9. Tamping the coffee blocks the water, raises the pressure and produces burnt coffee. The fix: fill the funnel and level it with your finger, nothing else. The whole method is in the moka pot guide.
Maintenance mistakes
11. Not descaling
Limescale cuts the flow, lowers the real water temperature and eventually burns out the pump. The fix: every 3 months, or every 2 with hard water.

12. Not cleaning the coffee oils
Coffee oils oxidise and turn rancid. That "stale" taste that suddenly appears almost always comes from the portafilter, the moka pot filter or the carafe, not from the bean.
